How can an AI incident answer mislead without an explicit false claim?

An answer can omit the condition that makes its conclusion usable, even when no individual sentence is an obvious falsehood. The PHP2Shell study labelled 23 of 148 eligible answers potentially misleading in its 23 October 2026 collection. Read that label as a reason to inspect context, not as a count of exploited systems or demonstrated harm.

What we measured

Measured. PHP2Shell's published study retained 148 eligible answers from 150 planned observations on 23 October 2026, with two exclusions. Its method assessed captured claims and cited sources against dated reference evidence. Reviewers labelled 23 answers potentially misleading and identified two answers with a wrong fixed version. This page uses the published labels rather than recoding them into a new error or harm rate. A potentially misleading answer and a factually wrong version describe different review questions; the counts should not be added as if they were disjoint groups.

What this means for you

Reasoned. Read the implied decision as well as the literal sentence. A statement about an incident may leave out the product boundary, the date of the evidence or whether a quoted change was proposed or completed. A reader can then take an action that the source does not support, without finding a plainly false sentence to underline.

Use a short comparison record. Save the answer's exact conclusion, the primary passage it relies on and the condition the reader would need to apply it. If the answer does not supply that condition, record it as missing rather than filling the gap with your own assumption. If the primary source also leaves it unknown, the responsible conclusion remains unknown.

Do not combine every issue into one error label. A wrong version is a specific factual discrepancy; an incomplete explanation has a different defect. The study's two wrong fixed-version answers and its potentially misleading classification are not interchangeable categories. Their relationship should be read from the published records, not assumed to be separate groups or added into a new total.

This approach lets a reviewer explain the practical problem without claiming knowledge of the model's intent. It also gives a publisher a precise source-backed correction instead of a general complaint that the answer feels unsafe.

  • For a vendor: make the product, date and state visible beside the incident claim.
  • For an answer reviewer: identify the unsupported reader inference and preserve the source passage.
  • For a buyer: seek the missing condition from a primary source before treating the summary as a technical verdict.
